The pus and debris from the infection builds up and compresses the lung. Pleurisy by itself doesn’t cause a collapsed lung. That’s “empyema” not emphysema. Just to make that clear. Some people get them mixed up. But they are two totally different things. Another cause is like if you were to lie in the same position for days at a time you would get what’s called atelctasis. The lung would collapse from not being ventilated. This is called hypostatic pneumonia.
