When are patient care conferences held?

A post-admission conference (PAC) is scheduled two weeks after admission. At this 15-minute conference, family members have an opportunity to discuss the resident’s plan of care with care team members (the physician does not attend this meeting). If the family cannot attend, the social worker will review the plan with you. Regular patient care conferences (PCC) take place every three months. Again, social workers keep families informed as to dates and times.
