Where can I find the SF-1199A Direct Deposit Sign-Up Form and the Primary Contact Form?
Both of these forms are located on the Division of Payment Management’s (DPM) website at www.dpm.psc.gov. You will then need to click on the Grant Recipient Information link located on the left side of the screen and then click on the Forms link. To access the SF-1199A Direct Deposit Sign-Up Form, click on the HHS Grantee Banking Information SF-1199A link. To access the Primary Contact Form, click on the Primary Contact Information link.
