Is this allowed and if so, how do I proceed? A. The majority of parents register their children online for the school based immunization program. With the approval of the District / School, we will offer paper student consent forms for those parents who are unable to complete the registration online. You will need to fill out and sign the student consent form and provide payment and/or insurance information. The School Administration will determine if you will be permitted to attend the school clinic event with your child or not. We will be prepared to vaccinate students with both online consent forms
