Your training program and the EMS Agency have the required forms and location of sites in Santa Clara County. Certification requests can not be processed until the EMS Agency has received your record from the Department of Justice. It is in your best interest to complete the LiveScan process as soon as possible. You are NOT required to have completed the EMT program to start the LiveScan process.
