Where do I send I-9 and W-4 forms?
Upon completion, I-9 and W-4 forms should be sent to the Payroll Office at Box 355655. However, you should check with your organizational unit to see if there are any additional steps within your unit. Federal law requires that employers complete the I-9 within the first three business days of employment. When a department is entering a new employee or rehiring a separated employee in OPUS, the citizenship field in HEPPS is automatically cleared. When Payroll receives the I-9 form, the citizenship information will be entered in the HEPPS database. Using OPUS, departments will have the ability to query and get a list of people in their department for whom the I-9 is missing. In the event that Payroll does not receive an I-9, a follow-up call to the administrator will be made. If Payroll does not receive a W-4, the IRS requires that we withhold at the maximum amount, zero allowances and marital status single.
