Quito is a great place to stay to have easy access to the activities you are interested in and more. Tour guide Angel Rea does all of the above from Quito. There is bird watching at the Parque Metropolitano in Quito, and the jungle and rainforest are very close. Also, Quito is between Mindo and Banos. Banos is the portal of the jungle and has lots of waterfalls and tame river rafting AND bunjee jumping. Horseback riding is nearby Quito also. There are daily flights to the Galapagos.
