You are required to submit the Hang Seng Mandatory Provident Fund – SuperTrust Direct Debit Authorisation (Employer) form [HA04], the Hang Seng Mandatory Provident Fund – SuperTrust Plus Direct Debit Authorisation (Employer) form [HA14] or the Hang Seng Mandatory Provident Fund – SimpleChoice Direct Debit Authorisation (Employer) from [HAE4] if you want to set up Direct Debit as the payment method.
