Starting December 14, 2009 people over 6 months of age who are: SCCA patients* (with an order from your clinical care team) • SCCA donors • Caregivers and household members of SCCA patients *If you are undergoing bone marrow transplantation you should not receive the flu vaccine. You should consult with your care team if you have any questions. Vaccine Clinic Dates and Times: Starting Monday, December 14, 2009, clinics will be held Monday through Thursday from 8 A.M. to 2 P.M. • No appointments necessary. Vaccine will be given on a first-come, first-serve basis. Vaccination Costs: We will bill insurance for all SCCA registered patients; please bring your SCCA card. • For those who are not SCCA registered patients, there will be an administration charge of $15 (cash or check only). A receipt will be provided to you, if you wish to submit it to your insurance company for reimbursement.
