This is a common occurrence for my friends, she is genetically very thin. People usually think this because they are jealous or ignorant. People who have anorexia look unhealthy, their hair and nails become dry and brittle, their skin is dull and ashy and they look weak and frail. People who are just genetically thin have a good color in their face and usually a little bit of muscle mass and are more proportionate. So people who think all thin people are automatically anorexic are probably just self conscious and want to make themselves feel better by thinking that the thin is miserable and has an eating disorder.
