If the downloaded PDF document does not contain the correct information then you did not enter the correct information during the registration process. All of the information in the downloaded documents is pulled directly from the registration forms. When I try to download the Self Certification / Tax Document I get a blank screen. If you get a blank screen when attempting to download these forms then chances are you do not have the latest version of the Adobe Reader. If you are still unable to download the forms after installing the latest Adobe Reader, please contact Technical Support at (713)439-0777.
