Why is “gridiron incision” so called?
A gridiron incision is a muscle-splitting incision. It is always desirable to part the fibres of a muscle to go through rather than cut across the fibres, but it gets complicated in the abdominal wall where the layers of muscle run in different directions. The gridiron incision proceeds by splitting each muscle in turn in the direction in which the fibres run.
