A product qualifier is a required data element for the ASC X12N 837P 4010A1 electronic format transaction and it should also be entered on the CMS-1500 claim form when submitting UPNs. Medi-Cal uses the product qualifier to determine that the medical supplies being billed for the UPN are appropriate.
