Would providing middle school counselors with information about a student’s elementary experiences help?
Ms. Kinney: Yes — and not only counselors, but teachers, as well. We have a transition form in our district that the fifth grade teachers to fill out. It gives us information on how the student is performing in each subject. It also describes the student’s study skills, as well his or her interests and hobbies. The form is designed to help us find out what we need to know to help each student be successful. It has been so invaluable that we use it with our high school transition, as well.
