A central goal of this FOA is to lessen health disparities, but the emphasis of this program should be on policy and environmental changes. Delivery of direct services is not within the scope of this announcement. Therefore, activities related to increasing access and using and enhancing delivery systems related to providing intensive case management could be supported through this funding announcement. For further reference see page 12, 13, 25, and 26 of the FOA for illustrations of the kinds of strategies that recipients may use to implement policy, environmental, programmatic, and infrastructure interventions.
