I heard that the red-shaded area on the last page of the paper survey will be used to identify my responses. What is the purpose of this red-shaded region?
The red-shaded region (with white circles) on the last page of the paper survey is used to identify the entity, department, or division; it’s the computer code equivalent of the department/division number. It allows the scanning software to accurately and efficiently assign the survey data to the appropriate group. It in no way identifies an individual.
