You are required to complete a ‘Change of Details of a Master Licence’ (P614B) form available from our website www.police.nsw.gov.au/sir or by requesting a form from the Customer Service Centre on 1300 362 001. You must send payment of the applicable fee with your application to the SIR. Please refer to the ‘Schedule of Fees’ for the appropriate fee. A replacement Master licence will be sent to you once your application is finalised.
