What is the sharing information question about?
We must ask your permission to share the information you give us about your child. The current form was printed in bulk before Social Services and Education were joined and became Children’s Services, so it shows a separate tick box for each. We decided to save money and use stocks of these forms rather than re-print them at this time. If you do not tick these two boxes, we cannot put your child’s name on the JIL. The third tick box gives your permission to share the information that you give us with Health.
