Disenrollment from our Plan means ending your membership with us. You can decide to leave for any reason during specified times (November 15 – December 31) or under certain special circumstances. TTY/TTD users should call 1-888-788-4010 to get a disenrollment form. • Call 1-800-MEDICARE at (1-800-633-4227) (TTY/TTD users should call 1-877-486-2048, 24 hours a day, seven days a week.
