To do that the following are needed: Childs full birth certificate in original along with a photocopy bearing parents particulars; fathers Pakistani passport; three passport size photographs, and completely filled-in endorsement application form also called form C. Most importantly, a written permission from the mother of the child, attested by a solicitor, allowing father to endorse the child on his passport.
