State applicants that have counties with populations of 500,000 people or more should exclude those counties from their application. If a city health department is located within a county with a population of 500,000 or more and would like to apply, they must either apply to serve the entire county including their city or apply in concert with the county health department. The city health department located in a county with a population of 500,000 or more should not be part of a state application. A funded applicant is responsible for implementing the program within the geographic area of the county and must serve the entire county including any cities within the county. Applicants applying to serve a large county must include letters of support from ALL health departments (e.g., city and county health departments, if they exist) located within and serving all or parts of the county AND a letter of support from the state health department.
