Please call the Denver Office of Disability Rights (DODR) office at 720-913-8480 Voice or 720-913-8492 TTY and an application form will be mailed to you. The form is available on the Motor Vehicle website at Denver Motor Vehicle Division and the DODR website. This form must be signed by a physician and submitted to your local county Motor Vehicle office as they issue the disability plate/placard.
