Applicants are required to attach self attested photocopies of all the documents with the application form. In case of submission at Citizen Service Centre (CSC), particularly at Bangalore-1, applicants are requested to attach attested photocopies (either gazetted official or notary) of all documents with the application form. Original documents must also be produced at the counter, which will be returned after verification.
