Do I need to send an Explanation of Benefits (EOB) form after every Neulasta® injection?
No. Ongoing eligibility in the program is contingent on submission of the EOB form that documents the patient’s first injection of Neulasta® covered by the program. The applicable EOB must be submitted within 30 days of such first injection. Failure to timely submit the required EOB may result in program ineligibility. By participating in this program, clinics/institutions agree to periodic audits related to administration of this program by Amgen. Your clinic or institution can fax the applicable EOB form to 1-888-653-2972.
