Do surgeons and practice employees have to qualify medically for voluntary life insurance coverage?
It depends on the amount of life and AD&D coverage each staff member chooses. In the Coverage Options table below, note the Guarantee Issue amount for employees, spouses and dependent children. All elections up to that Guarantee Issue amount are automatically approved. If you choose an amount greater than Guarantee Issue limit, it will require additional paperwork.
