A. Kansas does not require that a formal document be created for the purpose of documenting family input into the rating process. The Child Outcomes Summary Form provides a space for describing how and what information has been collected from the family to help in the rating process. It is at the discretion of the Infant/Toddler Network or Local Education Authority regarding how or in what format to collect that information.
