If you are a motorist that has been assisted by FSP, you should have been given either a postage-paid survey card to fill out and drop in the mail or a small card with a reference number to take an online survey. The online survey can be accessed by simply typing sac.fspsurvey.com into your browser’s address bar and hitting ‘Enter’ (you do not need to enter “www” or “http://”).
