You do not apply for an Italian passport, you apply for a visa. List of documents, downloadable application form, where to apply, cost of visa etc… you can find all the info here: http://www.esteri.it/visti/home_eng.asp You select Philippines as your nationality, Canada as your residence; choose the duration of your visit (up to 90 days), the reason of your visit (tourism), then click the “confirm” button.
