To enroll in an OptumHealth program, please call 888-842-4224. An OptumHealth representative will ask you which target biometric values you were unable to meet or for details about your chronic condition. (You will not need to share your exact biometric values.) If you are identified as qualifying to participate in a wellness or disease management program through your claims, an OptumHealth representative may also contact you.
