A. Complete the United Concordia Enrollment Form (#11 on the forms page of this website) and fax it to United Concordia at 800.329.9093. Otherwise, you can send a request to Christie Clemens via e-mail // or fax 610-230-2115. Be sure to include the employee name, ID # and effective date of termination.
