The following information must be provided to Mary Bennett, Dean’s Office, D4-4, either by e-mail (mbennett@dental.ufl.edu), fax (352-392-3070), or hard copy: • First-middle-last name • Credentials (e.g., D.D.S.) • Any other full name’s used (e.g., married name) • UF ID number • Date of birth • State of birth or country of birth • Gender • Social security number • Name and mailing address of program (e.g., Periodontology and address) • Telephone number of the program • Any numbers such as DEA, Medicare, Medicaid, etc.
