If you have exhausted all authorized visits given in the last 12 months, you should submit a Mental Health Care Services Request for Additional Visits (MHCSR), formerly known as the “Blue Form”. The MHCSR can be submitted electronically by logging in to www.tuftshealthplan.com/providers or by calling 800-208-9565 and using our Interactive Voice Response IVR system. There is one exception: If the start date of your most recent authorization is more than 12 months old, you can use the Mental Health Department’s secure Web site or the interactive voice response (IVR) system at (800) 208-9565 to request an Initial Authorization instead of completing the MHCSR. If the Tufts Health Plan member has unused visits authorized within the past 12 months, you will not have to submit a MHCSR until either the member has used almost all the authorized visits or 12 months have elapsed since the authorization began.
