How do I select a Sequoia Physicians Network primary care physician (PCP)?
If your HMO contracts with Sequoia Physicians Network during open enrollment, through your employer you may select a Sequoia Physicians Network provider. You can also select a Sequoia Physicians Network PCP at any time. First choose a participating provider within the Sequoia Physicians Network and contact your insurance company by calling the number listed on your card and inform them of the change.
