The office visit copays you pay under the new Employee Medical Plan will be determined using the same tier structure described above, so here’s what you’ll need to keep in mind when scheduling appointments: Under Tier 1, routine physical exams for you and your covered family members, along with most preventive screenings, are covered in full. All other office visits to Tufts Medical Center providers will be subject to a $10 copay per visit. Under Tiers 2 and 3, office visits to any physician who is part of the Tufts Health Plan network, but not affiliated with Tufts Medical Center, will be subject to a $30 copay per visit—with the exception of visits to specialists, which will each require a $50 copay. Lastly, under Tier 4, visits to out-of-network providers will be subject to an up-front annual deductible of $2,000 per person (or $4,000 per family).
