The MFLC will need information related to the success of the coordination aspect of this pilot project. Page 6 of the RFP refers to the collection of documents such as vision statements, Memoranda of Agreement and community work plans that reflect changes in service delivery. We will work with grantees to set up a protocol to collect and aggregate this information from communities.
