I have it installed locally to organize my work (apache on windows,php,mysql), I did all that, still, doesn work, cannot upload files larger than 5 or 6 megs, get a blank upload form. Ideas?

I had the same problem, tried everything above but it still didn’t work. Increase the ‘post_max_size’ variable in php.ini.
