I’m moving to a different province/country. What do I do about my Alberta Licence?
• Submit the Registration and Status Change and Physician Office Medical Records forms to the College. You will receive confirmation of receipt from the College. • Options: • Pay the registration fee for the next licensing period to keep your licence active • Licensing year is effective January 1 – December 31 • Voluntarily erase your name from the Alberta Medical Register.
