Is a PRIMIS+ scheme able to analyse its own data?
PRIMIS+ has developed a set of tools known as CHART that enables members of the practice team to see their own data in a variety of graphs and tables based on specific clinical topics, and identify individual patients who may need intervention. CHART is available through PRIMIS+ facilitators who can support and advise practices on its use. In addition to CHART, PRIMIS+ will continue to provide data analysis and feedback as part of its overall training and support service to primary care organisations.
