My child is in middle school, does he/she still need to fill out the Florida High School Athletic Association (FHSAA) Preparticipation Physical Evaluation Form (EL2)?

• Saint Andrew’s School is a member of the FHSAA and all Middle and Upper School students must submit this form prior to any participation in athletics. These forms will be given to your child by their coach at the start of their season. This form may also be accessed on this site by clicking here. In addition to the EL2 form, the FHSAA Consent and Release from Liability Certificate (EL3) must also be completed and returned to the coach. My child does not participate in a Saint Andrew’s School athletic activity (i.e. equestrian, dance, etc.). Does he/she still need to complete the Florida High School Athletic Association (FHSAA) Preparticipation Physical Evaluation Form (EL2)? • No. This form is only required for those middle school and upper school students that participate on a sanctioned Saint Andrew’s School athletic team.
