To obtain a new combination chart booklet, mail your request on your official letterhead along with the completed Chart Request Form. The cost for hardcopy charts or charts on disk is $50 (Note: Requests for full end user histories will be $100). Mail your request with your check or money order to the address below: Master Lock Company Attn. Customer Service/Chart Request 137 W Forest Hill Avenue PO Box 927 Oak Creek, WI 53154 Please allow 4 to 6 weeks from receipt of your letter to process your request.
