A. In order to order DME or certify home health services you must document the face-to-face care of the patient, which includes the need for the item or service. If the face-to-face encounter for home health certification was performed by a non-physician practitioner who is a member of UWP the physician will also have to document that he or she has reviewed the plan and agrees with the need for home health. A copy of the order form or certification form must also be kept in the medical record. The documentation requirements are the same whether or not the service is billed. The Home Health agency or DME supplier will also ask you to certify that you have seen the patient as part of your confirmation of the order.
