You must submit the following original documents: • FAA Form 8710-1 Part A: completed, signed, and dated • FAA Form 8710-1 Part B: completed • FAA Form 8710-1 Part C (only if you have a rural or PO Box address): completed • Official Identification Document: signed, dated, and notarized • Affidavit and Graduation Certificate: signed and dated (this document may be faxed; it does not need to be original) • Your original CFI certificate: make a photocopy of the front and back of your CFI certificate and the Official Identification Document to serve as your interim CFI certificate while you wait for your temporary certificate
