Along with the completed SLPCF Report and Rating form that the clinical fellow and mentoring SLP submit, the Clinical Fellow must submit the Employer(s) Verification Form. This document must be signed by the employer attesting to the completion of the 36-week full-time SLPCF or its part-time equivalent.
