If your relative has been in hospital for six months or more, they will have been given an individual questionnaire to complete by someone in the hospital, such as a manager. If they are only temporarily in hospital (and will have been there for less than six months) you should include them on your household questionnaire. If needed, another member of the family, a friend, or member of hospital staff can complete the questionnaire on their behalf.
