What if I am covered by Medicare, Medi-Cal or both?
Send a copy of your ID cars and a completed Signature Authorization Form (SAF) form so that the City of Long Beach can directly bill Medicare, Medi-Cal or both. The completed SAF Form should be sent to: City of Long Beach, 333 W. Ocean Blvd., ATTN: Ambulance Billing Section – Lobby Level, Long Beach, CA 90802.
