Based on web development timelines, some sections of the HRQ Web site might not reflect the deadline change to January 18, 2010 from September 16, 2009. You will qualify for the discounted monthly contribution rate for Motorola Health Plans (Health Investment Plan, Health Advantage Plan and Health Plan Plus) coverage in 2009 starting with your first February 2010 paycheck deduction if your confirmation page states that you have completed the Questionnaire before January 18, 2010 Midnight Central Time.
