Damaged Premium TransitChek MetroCards will be replaced by TransitCenter, upon receipt of a signed Certification form, and the damaged Card. Replacement cards issued for damaged Cards will be active upon receipt, and can be used immediately. To obtain a Certification Form contact us at enrollment@transitcenter.com with your mailing address.
