The assessment date is the date that you assessed the client for the service for which you are billing. When you are billing for a hearing aid repair, the assessment date is the date the client picked up the repaired aid and signed the Service Certificate. When you are billing for a replacement ear mold, the assessment date is the date the client picked up the ear mold and signed the Service Certificate.
