What is the Electronic Patient Record (EPR)?
A patient’s EPR includes all information electronically documented about a patient for the purpose of patient care. This includes pertinent facts on an individual’s health history, including all past and present health conditions, illnesses and treatments. It is related to the patient’s course of treatment while under the care of SJHC and/or LHSC, and includes assessments, investigations, diagnoses, plan of care or treatment and interventions. Having the information collected and stored online will allow access to the right patient information, at the right place, at the right time.
