We look at many factors in combination: overall GPA, science GPA, GRE scores, references, writing skill, and healthcare experience. Things that are likely to disqualify an applicant are poor science GPA, too many outstanding prerequisites, wanting to practice only in a big city or in a very specialized type of practice. We are looking for understanding of the PA role, communication and problem-solving skills, understanding of patients, understanding of health care delivery systems, leadership and experience, and ability to work as part of a team. These things we attempt to surmise from your essay and previous work experience, even if it has not necessarily been in the healthcare field.
